Laparoscopic Vertical Sleeve Gastrectomy

What is it?

Vertical sleeve gastrectomy is surgery to help with weight loss. The surgeon removes a large portion of your stomach.

Details

New smaller stomach is about the size of a banana. It limits the amount of food you can eat by making you feel full after eating small amounts of food.

This surgery is done under General Anesthesia. Surgeon puts 2 to 5 small cuts of 2 to 4 cm size each on belly through which laparoscope and other instruments can be passed in abdomen. Surgeon can view inside of abdomen through video monitor attached to laparoscope in operation room and can operate by viewing through it.

In this operation 75-80% part of the stomach is removed and remaining part is stapled to form a tube like structure (shape and size like a banana). Upper and lower sphincter muscles are left as it is in this operation through which food enters and leaves from stomach.

Definitions of Obesity

Obesity is defined by BMI (Body Mass Index)

BMI=Weight in kilograms/height in meters squared (kg/m2)

Class I obesity is defined as a body mass index >30 kg/m2

Class II obesity is a BMI >35 kg/m2

Class III (severe or morbid) obesity is >40 kg/m2

Class IV (super morbid) obesity is defined as >50 kg/m2.

The National Institute of Health Consensus Conference on obesity surgery recommends that surgery be considered for individuals who meet the following criteria:

Individuals with a BMI of 40 kg/m2 or greater

Individuals with a BMI of 35 kg/m2 or greater and who also have serious medical conditions (diabetes, obstructive sleep apnea) that would improve with weight loss.

How surgery helps in weight reduction:
  • Size of the stomach is reduced to about 25% so you feel full after little amount of food intake.
  • Stomach produces hormone ghrelin, which stimulates appetite. By reducing stomach size production of this hormone is also reduced. So appetite is also suppressed by this hormonal way.

How much weight loss is expected after surgery?

Weight loss after sleeve gastrectomy is faster and most of the patients loose 60 to 70% weight in first year after surgery.

Advantages of sleeve gastrectomy:

There is no major restriction to diet so dietary quality of life is good. You can eat anything but in small quantity. One should understand that this is not cosmetic surgery so behavioral and dietary modification is necessary to achieve the goal of desired weight loss after surgery.

As in other gastric bypass surgery, chances of malabsorption are minimal or nil in sleeve gastrectomy. Rapid and reliable weight loss can be achieved.
